Insurance Information
Get the best coverage for your treatments.
Dental insurance is an arrangement between you and your insurance company. We will file and process all claims at no additional charge. The office is a non-network provider from an insurance standpoint.
We make every effort to be certain your claims are filed accurately and quickly.
We cannot control or determine your insurance company's requirements or payments. Any disputes with your dental insurance must be handled by you, the insurance subscriber.
Financial Policy
Before you make that appointment...
It is your responsibility to keep your scheduled appointments.
We will make an effort to confirm your appointment 24-48 hours in advance by the method that is most convenient for you. If you are not available a message may be left on voice mail, e-mail or with another individual who answers the phone.
Missed appointments may be subject to a charge.
Payment for Services
Your prompt payment for services is appreciated. Timely payment allows us to offer your treatment at the most reasonable cost.
If you have dental insurance, your deductible and half of your estimated co-payment is due at the time services are rendered. Any remaining balance is to be paid within 60 days, regardless of insurance coverage.
Flexibility Counts
Arrangements Can Be Made!
If you are having financial difficulty and need to discuss your payment arrangements, please advise us at once so we can work with you to manage your account.
If you do not have dental insurance, payment at time of service is requested. For balances of more than $350, 1/3 is due at the time of service and the remaining balance within 60 days.
We offer Care Credit as a payment option for larger balances. Ask us for details.
Payment may be made by cash, check, Visa, MasterCard, Discover or American Express. Credit card payments are processed with Square.
